Based at: Client homes / service locations
Hours: Full-time / Part-time / Flexible / Shifts
Job Purpose
To provide high-quality, person-centred care and support to clients in their own homes, including those receiving Hospital at Home Services, ensuring their physical, emotional, and social needs are met. The Support Worker will work under the guidance of the Registered Manager and collaborate with healthcare professionals to deliver safe and effective care, following CQC Fundamental Standards.
Key Responsibilities
Personal Care & Daily Support
- Assist clients with personal care tasks (bathing, dressing, grooming,
toileting, mobility). - Support clients with meals, hydration, and dietary requirements.
- Aid clients in maintaining independence in daily activities.
- Administer medication according to care plan and company policy (if
trained/qualified).
Hospital at Home Services
- Monitor clients’ health status, including vital signs (temperature, blood pressure, pulse, oxygen saturation) under training and protocol.
- Observe and report clinical changes to Registered Manager or
supervising healthcare professional. - Support clients recently discharged from hospital, ensuring continuity of care and adherence to clinical instructions.
- Coordinate care with visiting nurses, therapists, and other healthcare providers.
- Document all clinical observations and interventions accurately in Nourish.
Client Wellbeing & Engagement
- Provide emotional support and companionship.
- Encourage participation in activities and social interaction.
- Respect clients’ preferences, dignity, and cultural choices.
Care Planning & Record Keeping
- Follow care plans, risk assessments, and Hospital at Home protocols accurately.
- Record all care and observations in Nourish.
- Report any changes in client health, behaviour, or wellbeing to the
Registered Manager promptly. - Log incidents, accidents, or safeguarding concerns according to company policy.
Health, Safety & Safeguarding
- Ensure a safe environment for clients, staff, and visitors.
- Adhere to infection control, manual handling, and health & safety
procedures. - Understand safeguarding responsibilities and report concerns immediately.
- Comply with Duty of Candour requirements when incidents occur.
Compliance & Professionalism
- Follow company policies and procedures at all times.
- Attend mandatory training, Hospital at Home protocols, and
supervisions. - Maintain confidentiality and comply with GDPR.
- Represent the company professionally to clients, families, and
healthcare professionals.
Person Specification:
Care Worker
Skills & Qualifications Required:
- Experience in care, support, or healthcare environment
- Empathy, patience, and strong communication skills
- Ability to follow care plans and accurately record observations
- Basic IT skills for care recording (Nourish or similar)
Desirable:
- QCF/Health & Social Care Level 2 or equivalent
- First Aid qualification
- Experience in Hospital at Home, post-discharge, or community nursing support
- Medication administration experience

CQC-Relevant Requirements - All care delivered must meet Fundamental Standards of Care (Safe, Effective, Caring, Responsive, Well-led).
- Accurate recording of care and clinical observations in Nourish is mandatory.
- Care Workers are part of a regulated workforce and must follow safeguarding, duty of candour, and health & safety procedures.
